SEVEN MIXED MARTIAL ARTS

PARTICIPANT WAIVER & RELEASE OF LIABILITY

Please read carefully before signing.

1. Acknowledgement of Risk

I understand that participation in mixed martial arts, Brazilian Jiu-Jitsu, grappling, wrestling, boxing, kickboxing, Muay Thai, strength and conditioning, fitness training, sparring and related activities involves inherent risks.

These risks may include, but are not limited to, bruises, cuts, sprains, strains, fractures, dislocations, concussion, head or spinal injury, illness, permanent disability and, in rare circumstances, death.

I voluntarily choose to participate with knowledge of these risks.

I acknowledge and accept the inherent risks of participation.

2. Health & Fitness Declaration

I confirm that, to the best of my knowledge, I am physically capable of participating in the activities I choose to undertake.

I agree to inform SEVEN MMA and its instructors of any injury, medical condition or other circumstance relevant to my safe participation.

I understand that I am responsible for deciding whether I should seek medical advice before participating and that I should stop training and notify an instructor if I become injured or unwell.

I confirm that I have read and understood this health declaration.

3. Rules & Safety

I agree to follow the gym's safety rules and reasonable instructions given by coaches and staff.

I understand that I must train responsibly, respect my training partners, stop immediately when a partner taps or verbally submits, and notify a coach of unsafe conditions or injuries.

I agree to follow the gym's rules and safety instructions.

4. Release & Limitation of Liability

To the fullest extent permitted by applicable law, I acknowledge and accept the ordinary and inherent risks associated with voluntarily participating in training at SEVEN MMA.

I understand that nothing in this waiver is intended to exclude or restrict any liability, duty, right or remedy that cannot lawfully be excluded or restricted.

5. Personal Property

I understand that I am responsible for my personal belongings while on the premises and that SEVEN MMA is not responsible for loss, theft or damage except where liability cannot legally be excluded.

6. Privacy

Personal information submitted through this form will be handled in accordance with UK data protection laws.

7. Participant Agreement

I confirm that I have read this document, had the opportunity to ask questions, and understand the acknowledgements and agreements I am making.

I have read and agree to the Participant Waiver & Release of Liability.

For Participants Under 18

 IF YOU ARE A GUARDIAN FILLING THE FORM FOR AN UNDERAGE MEMBER.

I am the participant's parent or legal guardian. I have read this agreement and, to the extent permitted by applicable law, consent to the minor's participation and agree to the applicable terms on their behalf.
